Tower of Pain is a mod for Heretic, created by Udderdude "Christopher Emirzian".
Description (in author’s own words):
The Tower Of Pain holds a sacred artifact that you must recover. All who have tried before you have met an untimely end. Will you succeed where they have failed?
I made this map when I realized there's not really any 'insane' Heretic maps with hundreds of enemies. Well, here it is. Don't play on UV unless you like alot of monsters ;)
