You receive this quest while completing the side quest The Thrill of the Chase. You need to find clues in Adam's house which suggest that you should explore the quest area. The location in question is the cemetery in the Eastern part of the map. Clear the cemetery of the zombies and find the tomb of Tom Freeman - it has been shown in the above screenshot.
Approach the plaque attached to one of the tomb's sides and interact with it to rip it off. Inside the tomb you will find a Note and a Saber which you need to complete The Thrill of the Chase.
