The eponymous Antaeus, the last super-cruiser, is also the last hope of humanity. The ship, which contains e.g. a nan-factory producing various battle units, sails out for the last time to infiltrate the suspicious archipelago and destroy the enemy headquarters. Hostile Waters: Antaeus rising features 21 missions, during which you lead various war machines such as tanks, helicopters, or planes. You can issue commands to up to fifteen units at the same time, but you can also assume direct control over a given machine. There is also the picture-in-picture mode, which allows you to watch the situation from multiple places on the battle field at once.

It turns out the protagonist's employer has earned himself the dislike of both Russian and Italian mobsters, and the player's task is to get Tony out of this troublesome situation. A number of new locations was created for the sake of the expansion, including night clubs, golf courses, an underground boxing ring and more. The expansion features mainly colorful story missions. During these, the protagonist not only drives vehicles or shoots enemies, but also performs such breakneck acts as blowing up a yacht, stealing a car from a moving train, or parachuting off a skyscraper. Additionally, the expansion offers a variety of side activities and mini-games, such as dancing at the clubs, and introduces new weapons and vehicles to the game.

The player once again assumes the role of Gabriel Belmont, who decides to help the Queen of Vampires by the name of Laura - a former subordinate of Carmilla, whom Gabriel defeated in the base game. Both the characters set out for a dangerous journey, during which they encounter powerful, dark forces awakened after the defeat of Lords of Shadow. What was not the case in the core game, is an opportunity to play as a different character - the expansion puts the player in the shoes of Laura as well. Contrary to the base game featuring real-time cutscenes, the expansion offers an impressive equivalent in the form of an animated comic strip.
