New achievements and trophies introduced in Blood and Wine The Witcher 3: Blood and Wine Guide
The chapter dealing with achievements and trophies in The Witcher 3: Blood and Wine contains a list of all the new achievements as well as tips on how to collect them. The trophies include: Weapon "W", David and Golyat, A Knight to Remember, or Playing House, among others.

Important - Achievements are common to all platforms, namely PCs (Steam achievements), Xbox One, and PlayStation 4 (Trophies).

Last Action Hero
Description: You need to be awarded the Vitis Vinifera medal. Geralt can receive this award in one of the final quests of the main story - Pump and Strange Circumstance. It will only happen if the protagonist killed Dettlaff (main quest Tesham Mutna) and he didn't allowed Syanna to die. More details about it can be found in the Endings chapter.
Weapon "W"
Description: You have to develop a single mutation of your choice. A description of their effects can be found in the section Mutations in this chapter of the guide. Developing mutations becomes possible after reaching the secret laboratory in the course of the side quest Turn and Face the Strange.
David and Golyat
Description: This is one of the hidden achievements. You must defeat the Golyat by hitting him in the eye with the crossbow. Try saving the game before starting the battle with the golyat. Reduce most of his health bar and start using the manual aiming mode of the crossbow. You have to aim at the hole in the barrel that the Golyat has on its head. This way you can hit it in the eye. The encounter takes place during The Beast of Toussaint main quest.
Hasta la Vista
Description: This is one of the hidden achievements. You receive it for killing a frozen opponent with a crossbow shot. You can freeze an opponent for example by using the "Piercing Cold" mutation's effect.
The Grapes of Wrath stomped
Description: This is one of the hidden achievements. You receive it for reconciling the quarreled families of de Coronata and Vermentino. It will happen during the Wine Wars series of side quests. You have to finish the quest Wine Wars: Belgaard, making Liam de Coronata and Matilda Vermentino manage the vineyard together. Once you deal with the vineyard's problems, you will be asked to wait three days for a special reward. The reward is a bottle of a rare wine. The achievement will be unlocked shortly after receiving this gift.
Turned Every Stone
Description: You have to find all the grandmasters' schematics of all the witcher schools (wolven, griffin, ursine and manticore). Lists of grandmaster diagrams can be found in the New Additions in Blood and Wine chapter and in the Grandmaster Witcher Gear quests part of the walkthrough.
Dressed to Kill
Description: You need to unlock the bonus for collecting a set of gear from any of the witcher schools.
A Knight to Remember
Description: You have to achieve perfect score in all competitions during the tourney. Participation in the tourney is part of one of the bigger side quests in the expansion - The Warble of a Smitten Knight. For the purpose of this achievement Geralt has to successfully pass four competitions - archery, horse race, a large arena battle, and a duel with the arena champion. Detailed information can be found in the section dedicated to the description of the said quest.
Kling of the Clink
Description: You have to be put into prison while visiting Toussaint. In order to unlock this achievement Geralt's action must lead to the medium ending of the game - the death of Syanna by the hands of Dettlaff. It will happen if while visiting the fairy land (Beyond Hill and Dale quest) the protagonist won't obtain the magic ribbon which would later save the life of Syanna. After she dies Geralt will be put in prison (quest Burlap is the New Stripe). More details about it can be found in the Endings chapter.
I Have a Gwent Problem
Description: You need to collect all the Gwent cards from the Skellige deck. You can assemble the cards in the course of the side quest Gwent: Never Fear, Skellige's Here More information to be found in the Skellige deck section later in this chapter of our guide.
Playing House
Description: You must build all the extensions available in the Corvo Bianco Vineyard. Geralt receives the property deed during the main quest - The Beast of Toussaint. The side quest Corvo Bianco is associated with the expanding of your property. It consists mainly of meeting with the majordomo and paying for subsequent development projects. For more detailed information, please refer to the section Your own estate.
Embodiment of the Five Virtues
Description: You have to receive Aerondight sword from Lady of the Lake. You receive is as a reward for completing the There Can Be Only One side quest, where you have to complete five trials of virtue that you receive from the hermit. Once you complete the trials, you have to meet with the hermit again and defeat him in a honorable fight.
The Witcher's Gone South
Description: You need to reach the territory of the Toussaint duchy. The travel to this part of the game world is associated with the quest Envoys, Wineboys, which becomes available after you install the expansion.
