The game is focused entirely on the multiplayer Nazi Zombies mode from Call of Duty: World at War, in which the players team up to fight the oncoming waves of the living dead. The gameplay takes place in enclosed locations that can be prepared for the attack, e.g. by nailing up doors and windows.

In the game, we assume the role of a Unit 13 spec ops operator who, after waking up from cryogenic sleep, discovers that most of the crew is dead and the rest turned into bloodthirsty zombies.
