The plot presents the further fate of Kazum Kiryu, a former member of the title mafia organization. This time he has to alleviate the conflict between two influential clans. Developers from Sega remained faithful to the mechanisms of the original game, enriching it with new elements, including a large number of new moves and interesting mini-games.

For his punishment, he has to wear the Mask of the Accused, a living mask that brings him unending anguish.
